The clinic removes practitioners from its own portal. If they have not, ask them first — it is usually an oversight rather than a refusal. Where a clinic keeps displaying you after you have asked them to stop, raise it; continuing to advertise a practitioner who has left is a misrepresentation and is treated as one.
